ERISA Summary Plan Document*
The SPD (Summary Plan Document) is a critical piece of information to have when appealing an unjust insurance denial related to experimental/investigational claims, low reimbursement issues, or exclusions that may not truly exist. This following is an example of an SPD request letter that has proven useful in building a successful case for appeal. ERISA mandates receipt of an SPD within 30 days of the request (Statutory penalties exist for failure to comply). This communication should be directed to the employer, and sent certified or by fax to document receipt of the request. Despite the usefulness of this letter, it would be wise to consult a health care attorney familiar with ERISA statutes.

Duplex Ultrasound Imaging Of Lower Extremity Veins in Chronic Venous Disease, Exclusive of Deep Venous Thrombosis: Guidelines for Performance and Interpretation of Studies (revised November 2009)
These American College of Phlebology guidelines for lower extremity ultrasound venous imaging describe minimum standards for imaging protocols and reporting as well as qualifications for those individuals performing and interpreting these studies. The accuracy of the non-invasive venous study depends on the knowledge, skill and experience of the technologist or physician performing the studies, and the knowledge, skill and experience of the physician interpreting the studies.
ACP Guidelines for Varicose Vein Surgery (June 2008)
The following guidelines were written as a consensus document by Dr. Peter Benson, Chair of the Coding & Reimbursement Committee, and later edited by the Board of the American College of Phlebology. Representing the ACP's recommendation for a standard of care, these guidelines will be distributed to third party payers and other organizations or individuals who request it.
